In this role, you will collaborate with healthcare providers, social workers, and community organizations to ensure seamless transitions for patients while addressing their psychosocial and financial needs. With a focus on patient-centered care, you will play a pivotal role in enhancing the well-being of individuals and families during challenging times.
This is a full-time position with AWS (Alternate Work Schedule) and standard office hours, offering a competitive salary range of $2,400 - $3,000 SGD per month. Responsibility
- Coordinate and facilitate discharge planning for patients, ensuring smooth transitions to home or alternative care settings.
- Assess patients' and families' financial, emotional, and social needs, and connect them with appropriate resources.
- Collaborate with healthcare teams, including doctors, nurses, and therapists, to develop comprehensive care plans.
- Provide counseling and support to patients and families navigating healthcare challenges.
- Manage and document case files, ensuring compliance with healthcare regulations and organizational policies.
- Organize and lead support groups or educational workshops for patients and caregivers.
- Liaise with community organizations, government agencies, and insurance providers to secure necessary assistance for patients.
- Monitor and evaluate the effectiveness of care coordination services, making adjustments as needed to improve outcomes.
Qualifications
- A Bachelor’s degree in Social Work, Psychology, Counseling, or a related field; Master’s degree is a plus.
- Minimum 2-3 years of experience in healthcare social work, case management, or a similar role.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse stakeholders.
- Familiarity with healthcare systems, insurance processes, and community resources in Indonesia or Southeast Asia.
- Proven ability to manage multiple cases while maintaining attention to detail and organizational skills.
- Proficiency in Microsoft Office Suite and case management software.
- Empathy, patience, and a patient-centered approach to care.
- Fluency in English; knowledge of Bahasa Indonesia is an advantage.
