Responsibility
- Conduct comprehensive psychosocial assessments for patients and families to identify needs, risks, and strengths.
- Develop and implement individualized care plans in collaboration with healthcare teams, patients, and families.
- Provide counseling and emotional support to patients and families navigating health crises, chronic illnesses, or end-of-life care.
- Advocate for patientsā rights and access to medical, financial, and social resources, including community programs and government assistance.
- Facilitate support groups and educational workshops to promote mental health and coping strategies.
- Coordinate discharge planning and ensure seamless transitions between healthcare settings and home care.
- Document all interventions, progress notes, and outcomes in compliance with ethical and legal standards.
- Collaborate with local NGOs, hospitals, and government agencies to expand support networks for underserved populations.
Qualifications
- Bachelorās or Masterās degree in Social Work, Psychology, or a related field (accredited by relevant Indonesian or international bodies).
- Minimum 5 years of experience in medical social work, preferably in hospitals, hospices, or community health settings.
- Licensed or registered as a Social Worker in Indonesia (or eligible for registration).
- Proven ability to conduct psychosocial assessments and develop evidence-based care plans.
- Strong communication and interpersonal skills, with fluency in English and Bahasa Indonesia.
- Experience working with diverse cultural and socioeconomic backgrounds in Bali or similar contexts.
- Knowledge of local healthcare systems, social services, and legal frameworks in Indonesia.
- Commitment to ethical practice, confidentiality, and patient advocacy.
