Responsibility
- Provide clinical supervision and mentorship to a team of social workers, ensuring high-quality case management.
- Lead psychosocial assessments and develop comprehensive care plans for patients with complex medical and social needs.
- Coordinate with multidisciplinary teams, including doctors, nurses, and allied health professionals, to facilitate seamless patient transitions.
- Represent the organization in community outreach programs and foster strategic partnerships with external social service agencies.
- Monitor and evaluate social work intervention outcomes to ensure continuous improvement in service delivery.
- Manage complex crisis intervention cases and provide support to families navigating end-of-life care or long-term health transitions.
- Ensure compliance with healthcare regulations, ethical standards, and documentation protocols within the department.
Qualifications
- Bachelor’s Degree in Social Work or a recognized Graduate Diploma in Social Work (accredited by SASW).
- Minimum 5-7 years of clinical experience in a healthcare or hospital setting.
- Proven leadership experience in supervising staff or managing departmental social work teams.
- Registered Social Worker (RSW) status with the Singapore Association of Social Workers (SASW).
- Strong analytical skills with a focus on evidence-based practice and case management.
- Excellent communication and interpersonal skills, with the ability to advocate effectively for patients.
- Proficiency in local languages and dialects to effectively communicate with a diverse patient demographic.
