Responsibility

  • Conduct comprehensive telephonic and virtual patient assessments to evaluate health status, identify care needs, and develop individualized care plans.
  • Coordinate with multidisciplinary healthcare teams (physicians, specialists, social workers) to ensure seamless transitions of care and adherence to treatment protocols.
  • Monitor patient progress through regular follow-ups, adjusting care plans as needed to address evolving health concerns or barriers to compliance.
  • Educate patients and families on disease management, medication adherence, and preventive care using clear, empathetic communication.
  • Document all patient interactions, assessments, and interventions in EHR systems with accuracy and compliance to U.S. healthcare standards (e.g., HIPAA).
  • Identify high-risk patients and proactively intervene to prevent hospital readmissions or complications.
  • Collaborate with insurance providers and case managers to facilitate prior authorizations, referrals, and resource allocation.
  • Stay current with evidence-based practices and participate in ongoing training to enhance clinical and technical skills.

Qualifications

  • Active, unrestricted US RN license (required). Compact state license or multi-state licensure is a plus.
  • Minimum 2 years of clinical nursing experience, preferably in care coordination, case management, telehealth, or chronic disease management.
  • Proven ability to assess, plan, implement, and evaluate patient care in a remote or virtual setting.
  • Excellent written and verbal communication skills in English, with a patient-centered and empathetic approach.
  • Proficiency in EHR/EMR systems (e.g., Epic, Cerner) and Microsoft Office Suite.
  • Strong critical thinking and problem-solving skills to navigate complex patient scenarios independently.
  • Ability to work flexible hours (including evenings/weekends) to accommodate U.S. time zones.
  • Reliable high-speed internet connection and a quiet, professional home office setup.
