Responsibility
- Conduct comprehensive telephonic and virtual patient assessments to evaluate health status, identify care needs, and develop individualized care plans.
- Coordinate with multidisciplinary healthcare teams (physicians, specialists, social workers) to ensure seamless transitions of care and adherence to treatment protocols.
- Monitor patient progress through regular follow-ups, adjusting care plans as needed to address evolving health concerns or barriers to compliance.
- Educate patients and families on disease management, medication adherence, and preventive care using clear, empathetic communication.
- Document all patient interactions, assessments, and interventions in EHR systems with accuracy and compliance to U.S. healthcare standards (e.g., HIPAA).
- Identify high-risk patients and proactively intervene to prevent hospital readmissions or complications.
- Collaborate with insurance providers and case managers to facilitate prior authorizations, referrals, and resource allocation.
- Stay current with evidence-based practices and participate in ongoing training to enhance clinical and technical skills.
Qualifications
- Active, unrestricted US RN license (required). Compact state license or multi-state licensure is a plus.
- Minimum 2 years of clinical nursing experience, preferably in care coordination, case management, telehealth, or chronic disease management.
- Proven ability to assess, plan, implement, and evaluate patient care in a remote or virtual setting.
- Excellent written and verbal communication skills in English, with a patient-centered and empathetic approach.
- Proficiency in EHR/EMR systems (e.g., Epic, Cerner) and Microsoft Office Suite.
- Strong critical thinking and problem-solving skills to navigate complex patient scenarios independently.
- Ability to work flexible hours (including evenings/weekends) to accommodate U.S. time zones.
- Reliable high-speed internet connection and a quiet, professional home office setup.
