Responsibility
- Provide skilled nursing care to patients in their homes, including wound care, medication administration, and monitoring of vital signs.
- Develop and implement individualized care plans in collaboration with physicians, patients, and their families.
- Educate patients and caregivers on disease management, preventive care, and healthy lifestyle practices.
- Conduct regular assessments of patients' health status and adjust care plans as needed to ensure optimal outcomes.
- Coordinate with multidisciplinary teams, including physical therapists, occupational therapists, and social workers, to provide comprehensive care.
- Maintain accurate and up-to-date patient records in compliance with regulatory standards and company policies.
- Respond promptly to patient emergencies and provide appropriate interventions or referrals.
- Participate in ongoing training and professional development to stay current with best practices in home health care.
Qualifications
- Bachelor of Science in Nursing (BSN) or equivalent degree from an accredited institution.
- Valid and active Registered Nurse (RN) license in the Philippines or eligibility for licensure in Indonesia.
- Minimum of 1-2 years of clinical nursing experience, preferably in home health, community health, or acute care settings.
- Strong clinical skills, including wound care, IV therapy, and patient assessment.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients and their families.
- Compassionate, patient-centered approach to care with a commitment to ethical practice.
- Ability to work independently, manage time effectively, and prioritize tasks in a home-based setting.
- Proficiency in basic computer skills for documentation and reporting.
