Responsibility
- Conduct comprehensive psychosocial assessments of patients to identify their emotional, social, and environmental needs.
- Develop, implement, and monitor individualized care plans in collaboration with healthcare teams to support patient recovery and rehabilitation.
- Provide counseling and emotional support to patients and their families to help them cope with illness, disability, or end-of-life care.
- Facilitate referrals and linkages to community resources, including support groups, financial assistance programs, and home care services.
- Advocate for patients' rights and ensure they have access to necessary medical and social services.
- Educate patients and families on available healthcare options, treatment plans, and self-management strategies.
- Maintain accurate and confidential patient records in compliance with healthcare regulations and ethical standards.
- Participate in multidisciplinary team meetings to review patient progress and adjust care plans as needed.
Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited institution.
- Valid registration as a Medical Social Worker with the relevant professional body (e.g., Philippine Regulatory Commission or equivalent).
- Minimum of 2 years of experience in medical social work, preferably in a hospital or healthcare setting.
- Strong knowledge of psychosocial assessment tools, case management, and community resources.
- Excellent communication, interpersonal, and counseling skills, with the ability to work effectively with diverse populations.
- Proficiency in English; additional languages (e.g., Bahasa Indonesia) are a plus.
- Ability to work independently and as part of a multidisciplinary team in a fast-paced environment.
