As a Care Coordinator, you will play a pivotal role in improving patient outcomes by facilitating access to specialized care, educating patients on asthma management, and streamlining clinical workflows. Responsibility
- Coordinate patient care plans for asthma patients in alignment with the Singapore National Asthma Program guidelines.
- Liaise between healthcare providers, patients, and community resources to ensure continuity of care.
- Educate patients and families on asthma management, treatment adherence, and preventive measures.
- Monitor patient progress, track clinical outcomes, and adjust care plans as needed.
- Maintain accurate and confidential patient records in compliance with healthcare regulations.
- Organize and facilitate patient support groups, workshops, or outreach programs.
- Collaborate with multidisciplinary teams to streamline clinic operations and improve service delivery.
- Identify and address barriers to care, advocating for patient needs and access to resources.
Qualifications
- Diploma or Bachelor’s degree in Nursing, Public Health, Healthcare Administration, or a related field.
- Minimum 2 years of experience in healthcare coordination, patient advocacy, or clinical support roles.
- Strong knowledge of asthma management protocols and chronic disease care models.
- Excellent communication and interpersonal skills to engage with diverse stakeholders.
- Proficiency in electronic health records (EHR) and Microsoft Office Suite.
- Ability to work independently and collaboratively in a fast-paced clinic environment.
- Certification in Basic Life Support (BLS) or equivalent is a plus.
- Fluency in English; additional proficiency in Indonesian or local dialects is advantageous.
