Responsibility
- Coordinate patient care plans for individuals enrolled in the National Asthma Program, ensuring adherence to clinical guidelines and best practices.
- Liaise between patients, healthcare providers, and community organizations to facilitate access to asthma education, medications, and follow-up care.
- Conduct patient assessments to identify barriers to asthma management and develop personalized intervention strategies.
- Organize and lead patient education workshops on asthma triggers, medication adherence, and emergency action plans.
- Maintain accurate and confidential patient records, tracking progress and outcomes in compliance with healthcare regulations.
- Collaborate with clinic staff to streamline referral processes and improve care continuity for asthma patients.
- Monitor program performance metrics and contribute to reports for stakeholders and funding bodies.
- Stay updated on the latest asthma treatment protocols and public health initiatives to ensure evidence-based practice.
Qualifications
- Diploma or Bachelor’s degree in Nursing, Public Health, Healthcare Administration, or a related field.
- Minimum 2 years of experience in healthcare coordination, patient advocacy, or a clinical setting, preferably in respiratory care.
- Strong knowledge of asthma management principles and public health programs.
- Excellent communication and interpersonal skills to engage with diverse patient populations and healthcare teams.
- Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
- Ability to work independently and collaboratively in a fast-paced, multicultural environment.
- Certification in asthma education (e.g., AE-C) is a plus.
- Fluency in English; knowledge of Bahasa Indonesia is advantageous.
