Responsibility

  • Conduct comprehensive psychosocial assessments for patients and their families to identify needs, risks, and support systems.
  • Develop and implement individualized care plans to address emotional, financial, and social challenges.
  • Provide counseling and crisis intervention to patients and families coping with illness, trauma, or end-of-life care.
  • Collaborate with healthcare teams, including doctors, nurses, and therapists, to ensure integrated patient care.
  • Advocate for patients’ rights and access to healthcare, social services, and community resources.
  • Facilitate support groups, educational workshops, and discharge planning to promote patient independence and well-being.
  • Document all interventions, assessments, and follow-ups in compliance with hospital and regulatory standards.
  • Stay updated on local and national social services, policies, and resources to provide accurate and timely referrals.

Qualifications

  • Bachelor’s or Master’s degree in Social Work, Psychology, or a related field from an accredited institution.
  • Minimum of 2 years of experience in medical social work, healthcare settings, or a similar role.
  • Licensed or certified as a Social Worker (preferred but not mandatory, depending on local regulations).
  • Strong knowledge of psychosocial assessment tools and intervention strategies in healthcare.
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse patient populations.
  • Proven ability to work in a multidisciplinary team and manage complex cases with empathy and professionalism.
  • Familiarity with local social services, NGOs, and community resources in Bali or Indonesia.
