Responsibility
- Coordinate follow-up care for asthma patients enrolled in the Singapore National Asthma Program.
- Liaise with healthcare providers, patients, and families to ensure adherence to treatment plans.
- Monitor patient progress and document outcomes in electronic health records (EHR).
- Organize and facilitate patient education sessions on asthma management and prevention.
- Collaborate with medical teams to adjust care plans based on patient needs and feedback.
- Conduct regular check-ins with patients via phone, email, or in-person visits.
- Maintain accurate and confidential patient records in compliance with healthcare regulations.
- Identify and address barriers to patient compliance and follow-up care.
Qualifications
- Diploma or Bachelorâs degree in Nursing, Healthcare Administration, or a related field.
- Minimum 2 years of experience in healthcare coordination, patient care, or a similar role.
- Strong knowledge of asthma management, respiratory conditions, and patient education.
- Excellent communication and interpersonal skills to engage with patients and healthcare teams.
- Proficiency in using electronic health record (EHR) systems and Microsoft Office Suite.
- Ability to work independently and manage multiple priorities in a fast-paced environment.
- Compassionate and patient-centered approach with a commitment to improving healthcare outcomes.
- Fluency in English; additional languages (e.g., Indonesian, Mandarin) are a plus.
