Responsibility
- Conduct comprehensive psychosocial assessments for patients and families to identify needs, risks, and strengths.
- Develop and implement individualized care plans in collaboration with healthcare teams, ensuring alignment with medical treatments.
- Provide crisis intervention, counseling, and emotional support to patients and families facing medical challenges.
- Facilitate access to community resources, including financial assistance, housing, and social services.
- Advocate for patient rights and coordinate with external agencies to secure necessary support.
- Educate patients and families on coping strategies, healthcare navigation, and available social services.
- Document all interventions, progress notes, and referrals in compliance with hospital and regulatory standards.
- Participate in case conferences, team meetings, and professional development to enhance service delivery.
Qualifications
- Bachelor’s or Master’s degree in Social Work (accredited by a recognized institution).
- Minimum of 2 years of experience in medical or healthcare social work, preferably in a hospital setting.
- Licensed or registered as a Social Worker (or equivalent certification in Indonesia).
- Strong knowledge of psychosocial assessment tools and evidence-based interventions.
- Excellent communication and interpersonal skills, with the ability to work with diverse populations.
- Familiarity with local community resources and social service systems in Bali.
- Ability to handle sensitive situations with empathy, discretion, and professionalism.
- Proficiency in English and Indonesian (written and verbal).
