Responsibility
- Conduct psychosocial assessments to identify patient needs, strengths, and support systems.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Provide counseling and emotional support to patients and families coping with illness, injury, or end-of-life care.
- Advocate for patients’ rights and access to medical, financial, and social resources.
- Facilitate support groups and educational workshops for patients and caregivers.
- Coordinate with hospitals, clinics, and community organizations to ensure seamless care transitions.
- Document patient progress and maintain confidential case records in compliance with healthcare standards.
- Stay updated on best practices in medical social work and integrate evidence-based interventions.
Qualifications
- Bachelor’s or Master’s degree in Social Work, Psychology, or a related field (accredited by relevant bodies).
- Minimum 2-3 years of experience in medical or healthcare social work.
- Strong knowledge of psychological support techniques and crisis intervention strategies.
- Excellent communication and interpersonal skills to build trust with diverse patient populations.
- Familiarity with healthcare systems, policies, and ethical standards in clinical settings.
- Ability to work independently and collaboratively in fast-paced environments.
