Responsibility
- Conduct psychosocial assessments to identify patient needs and develop individualized care plans.
- Provide emotional support and counseling to patients and their families during medical crises.
- Coordinate with healthcare teams to ensure comprehensive and patient-centered care.
- Advocate for patient rights and facilitate access to necessary medical and social services.
- Connect patients with community resources, such as financial aid, housing, and support groups.
- Document patient interactions and maintain accurate records in compliance with hospital policies.
- Participate in case conferences and multidisciplinary team meetings to discuss patient progress.
- Educate patients and families on available resources and coping strategies for managing health conditions.
Qualifications
- Bachelor's or Master's degree in Social Work or a related field.
- Licensed or certified as a Social Worker in Indonesia or equivalent qualification.
- Minimum of 2 years of experience in medical or healthcare social work.
- Strong interpersonal and communication skills to interact effectively with patients, families, and healthcare professionals.
- Empathy, compassion, and a commitment to patient advocacy.
- Ability to work collaboratively in a fast-paced healthcare environment.
- Familiarity with local community resources and social service programs.
- Proficiency in both English and Bahasa Indonesia is preferred.
