Responsibility
- Conduct psychosocial assessments to identify the needs and concerns of elderly residents.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Provide emotional and social support to residents and their families.
- Coordinate with community resources and services to ensure comprehensive care.
- Maintain accurate and up-to-date documentation of resident progress and interventions.
- Facilitate support groups and educational programs for residents and families.
- Advocate for residents' rights and ensure their voices are heard in care decisions.
- Participate in multidisciplinary team meetings to review and enhance care strategies.
Qualifications
- Bachelor's degree in Social Work, Psychology, or a related field.
- Minimum of 2 years of experience in medical social work or geriatric care.
- Strong understanding of psychosocial assessment and case management principles.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team environment.
- Compassionate and empathetic approach to patient care.
- Familiarity with local community resources and services in Bali.
