Responsibility
- Conduct comprehensive psychosocial assessments of patients and families to identify emotional, social, and financial needs in medical settings.
- Develop and implement individualized care plans in collaboration with healthcare teams, including doctors, nurses, and therapists.
- Provide crisis intervention, grief counseling, and emotional support to patients and families facing medical diagnoses, trauma, or end-of-life care.
- Facilitate patient advocacy by connecting individuals with community resources, financial assistance programs, and support groups.
- Educate patients and families on coping strategies, treatment adherence, and navigating healthcare systems.
- Document patient interactions, progress notes, and care plans in compliance with healthcare regulations and organizational policies.
- Collaborate with interdisciplinary teams to ensure holistic, patient-centered care across hospitals, clinics, and community health programs.
- Participate in case conferences, staff meetings, and professional development opportunities to enhance clinical skills and knowledge.
Qualifications
- Bachelorâs or Masterâs degree in Social Work (BSW/MSW) from an accredited institution.
- Valid social work license (e.g., LSW, LCSW, or equivalent) with eligibility to practice in Indonesia.
- Minimum of 2 years of experience in medical social work, healthcare settings, or related fields.
- Strong knowledge of psychosocial assessment tools, crisis intervention techniques, and case management principles.
- Excellent communication, active listening, and interpersonal skills to build rapport with diverse populations.
- Ability to work effectively in fast-paced, multicultural environments with sensitivity to cultural and religious practices.
- Proficiency in English; fluency in Bahasa Indonesia or other local languages is a plus.
