Responsibility
- Provide comprehensive nursing care to patients in their homes, including wound care, medication administration, and vital sign monitoring.
- Develop and implement personalized care plans in collaboration with physicians and other healthcare professionals.
- Educate patients and their families on disease management, medication adherence, and healthy lifestyle practices.
- Monitor and document patient progress, reporting any changes in condition to the healthcare team.
- Assist with activities of daily living (ADLs) and provide emotional support to patients and their families.
- Ensure compliance with healthcare regulations, infection control protocols, and safety standards.
- Coordinate with other healthcare providers to ensure seamless continuity of care.
- Maintain accurate and up-to-date patient records using electronic health record (EHR) systems.
Qualifications
- Valid nursing license (RN or equivalent) with a minimum of 2 years of clinical experience, preferably in home care or geriatric nursing.
- Strong knowledge of medical-surgical nursing, wound care, and chronic disease management.
- Excellent communication and interpersonal skills to build trust with patients and their families.
- Ability to work independently and make sound clinical judgments in a home care setting.
- Proficiency in using electronic health record (EHR) systems and basic medical equipment.
- Compassionate, patient-centered approach with a commitment to ethical nursing practices.
- Flexibility to adapt to varying schedules and patient needs.
- CPR/BLS certification and familiarity with emergency response protocols.
