Responsibility
- Conduct structured surveys and assessments with asthma patients to gather comprehensive health data and feedback.
- Collaborate with healthcare providers to develop and implement personalized care plans for asthma patients.
- Monitor patient progress and adherence to treatment plans, identifying areas for improvement and intervention.
- Analyze survey data and patient records to identify trends, risks, and opportunities for enhancing care quality.
- Coordinate with multidisciplinary care teams to ensure seamless communication and continuity of care for patients.
- Provide education and support to patients and their families on asthma management, medication adherence, and lifestyle modifications.
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations and organizational policies.
- Prepare reports and presentations for healthcare teams, highlighting key findings and recommendations for care optimization.
Qualifications
- Bachelorās degree in Nursing, Public Health, Healthcare Administration, or a related field (or equivalent experience).
- Minimum of 2 years of experience in healthcare coordination, patient care, or a related role, preferably in respiratory or chronic disease management.
- Strong understanding of asthma management, treatment protocols, and patient education strategies.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare providers.
- Proficiency in data collection, analysis, and reporting using healthcare software and tools (e.g., EHR systems, survey platforms).
- Detail-oriented with strong organizational and time-management skills to manage multiple patient cases efficiently.
- Ability to work independently and remotely while maintaining high standards of professionalism and accountability.
- Fluency in English; additional languages (e.g., Bahasa Indonesia) are a plus.
