Responsibility
- Conduct comprehensive assessments of clientsâ health and social needs to identify appropriate resources and support systems.
- Provide clear, empathetic guidance on available health benefits, social services, and community programs to eligible individuals.
- Develop and maintain up-to-date databases of local/regional health and social service providers, ensuring accuracy and accessibility.
- Facilitate referrals and coordinate care between healthcare providers, social workers, and community organizations.
- Organize and lead educational workshops or outreach sessions to promote health literacy and social welfare awareness.
- Monitor client progress and follow up to ensure continued access to necessary services and interventions.
- Collaborate with cross-functional teams to advocate for policy improvements or service gaps affecting client wellbeing.
- Document all interactions, assessments, and outcomes in compliance with organizational and regulatory standards.
Qualifications
- Bachelorâs degree in Social Work, Public Health, Psychology, Nursing, or a related field. Masterâs degree is a plus.
- Minimum 2 years of experience in healthcare coordination, social services, or community outreach programs.
- Strong knowledge of health and social benefit systems, preferably in Indonesia or Southeast Asia.
- Excellent communication and interpersonal skills, with the ability to engage diverse populations with cultural sensitivity.
- Proven ability to manage caseloads, prioritize tasks, and work independently in a fast-paced environment.
- Proficiency in Microsoft Office Suite and case management software; familiarity with CRM tools is advantageous.
- Fluency in English (written and spoken); proficiency in Bahasa Indonesia is highly desirable.
