Responsibility
- Assist medical social workers in conducting patient assessments to identify financial, emotional, and care-related needs.
- Provide direct support to patients and families, including guidance on available social services and resources.
- Help coordinate financial assistance programs, grants, or subsidies for eligible patients.
- Maintain accurate and confidential case records, ensuring compliance with healthcare regulations.
- Collaborate with healthcare teams to develop holistic care plans for patients.
- Facilitate referrals to external agencies, NGOs, or community programs as needed.
- Conduct follow-up visits or calls to monitor patient progress and adjust support plans.
- Participate in team meetings, case discussions, and professional development workshops.
Qualifications
- Diploma or Bachelor’s degree in Social Work, Psychology, Counseling, or a related field.
- Minimum 1-2 years of experience in social work, healthcare, or community service (fresh graduates with relevant internships are encouraged to apply).
- Strong empathy, active listening, and communication skills in English (additional proficiency in Indonesian or local dialects is a plus).
- Familiarity with case management, crisis intervention, and resource coordination.
- Ability to work independently and collaboratively in a fast-paced medical environment.
- Knowledge of healthcare systems, social welfare programs, or financial assistance schemes is advantageous.
- Proficiency in Microsoft Office, data entry, and basic administrative tasks.
