Responsibility
- Coordinate and facilitate access to healthcare services for patients, including scheduling appointments, arranging transportation, and linking clients with community resources.
- Conduct comprehensive assessments of patients’ medical, social, and emotional needs to develop personalized care plans.
- Liaise with healthcare providers, social workers, and insurance companies to ensure timely and appropriate care delivery.
- Monitor patient progress, follow up on treatment adherence, and adjust care plans as needed to optimize health outcomes.
- Educate patients and families on available healthcare services, preventive care, and chronic disease management.
- Maintain accurate and confidential patient records in compliance with healthcare regulations and organizational policies.
- Advocate for patients’ rights and needs, ensuring they receive equitable and culturally sensitive care.
- Collaborate with multidisciplinary teams to improve care coordination processes and enhance service efficiency.
Qualifications
- Diploma or Bachelor’s degree in Nursing, Social Work, Public Health, or a related field. Relevant certifications (e.g., Case Management) are a plus.
- Minimum 2-3 years of experience in healthcare coordination, patient advocacy, or community health services.
- Strong knowledge of healthcare systems, insurance processes, and community resources in Indonesia or Southeast Asia.
- Excellent communication and interpersonal skills, with the ability to build trust with diverse patient populations.
- Proficient in English and Indonesian (Bahasa); additional local dialects (e.g., Balinese) are advantageous.
- Demonstrated ability to manage multiple priorities in a fast-paced environment with attention to detail.
- Familiarity with electronic health records (EHR) and care management software.
