Responsibility
- Coordinate and manage patient care plans, ensuring all medical and non-medical needs are met.
- Act as a liaison between patients, families, and healthcare providers to facilitate effective communication.
- Monitor patient progress and adjust care plans as needed to achieve optimal outcomes.
- Ensure compliance with hospital policies, healthcare regulations, and ethical standards.
- Document patient information accurately and maintain up-to-date records.
- Provide emotional support and guidance to patients and their families throughout the treatment process.
- Collaborate with multidisciplinary teams to develop and implement comprehensive care strategies.
- Educate patients and families about treatment options, resources, and available support services.
Qualifications
- Bachelor's degree in Nursing, Social Work, or a related healthcare field.
- Minimum of 2 years of experience in case management, healthcare coordination, or a similar role.
- Strong knowledge of medical terminology, healthcare procedures, and patient care standards.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work effectively in a fast-paced, multicultural environment.
- Proficiency in using healthcare management software and electronic health records (EHR).
- Compassionate and patient-centered approach to care.
- Fluency in English and Bahasa Indonesia is preferred.
