Responsibility
- Develop and implement personalized care plans in alignment with UK healthcare standards and patient needs.
- Conduct thorough risk assessments to identify potential hazards and mitigate them proactively.
- Monitor and evaluate the quality of care delivery, ensuring adherence to regulatory and organizational guidelines.
- Collaborate with cross-functional healthcare teams to streamline care processes and improve patient outcomes.
- Prepare and maintain accurate documentation of care plans, assessments, and quality assurance reports.
- Identify areas for improvement in care services and recommend evidence-based solutions.
- Ensure compliance with UK healthcare policies, including CQC (Care Quality Commission) standards.
- Provide training and guidance to staff on best practices in care planning and quality assurance.
Qualifications
- Bachelor’s degree in Nursing, Healthcare Administration, or a related field (required).
- Minimum 3 years of experience in care planning, quality assurance, or a similar role within the healthcare sector.
- In-depth knowledge of UK healthcare regulations, including CQC standards and care planning frameworks.
- Strong analytical and problem-solving skills with a keen eye for detail.
- Excellent communication and interpersonal skills to liaise effectively with healthcare professionals and stakeholders.
- Proficiency in healthcare software and Microsoft Office Suite (e.g., Excel, Word).
- Ability to work independently and remotely with a high degree of self-motivation.
- Certification in Quality Assurance or Care Management (preferred but not mandatory).
