Responsibility
- Develop and implement personalized care plans for patients with chronic diseases, ensuring adherence to medical protocols and best practices.
- Coordinate with healthcare providers, including doctors, nurses, and specialists, to ensure comprehensive and continuous patient care.
- Educate patients and their families on disease management, medication adherence, and lifestyle modifications to improve health outcomes.
- Monitor patient progress through regular assessments, adjustments to care plans, and documentation of health status.
- Advocate for patients by liaising with insurance providers, social services, and community resources to secure necessary support.
- Conduct home visits or community-based care sessions to provide hands-on support and evaluate living conditions.
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations and organizational policies.
- Participate in multidisciplinary team meetings to review patient cases and contribute to continuous improvement in care delivery.
Qualifications
- Bachelor's degree in Nursing or a related healthcare field; registered nurse (RN) license is required.
- Minimum of 3 years of experience in clinical nursing, community health, or chronic disease management.
- Strong knowledge of chronic conditions such as diabetes, hypertension, and cardiovascular diseases.
- Excellent communication and interpersonal skills to build trust with patients and collaborate with healthcare teams.
- Ability to work independently and manage a caseload of patients with varying needs.
- Proficiency in electronic health records (EHR) and basic computer skills for documentation and reporting.
- Compassionate, patient-centered approach with a commitment to improving health outcomes.
- Fluency in English; knowledge of Bahasa Indonesia is a plus.
