Responsibility

  • Support the development and implementation of Advance Care Planning (ACP) initiatives in Singapore.
  • Collaborate with healthcare providers, community organizations, and government agencies to promote palliative care services.
  • Assist in the design and execution of training programs for healthcare professionals and caregivers.
  • Conduct research and analysis to identify gaps and opportunities in palliative care delivery.
  • Coordinate outreach and awareness campaigns to educate the public on the importance of ACP and palliative care.
  • Monitor and evaluate the effectiveness of programs and initiatives, providing recommendations for improvement.
  • Prepare reports, presentations, and documentation for stakeholders and senior management.
  • Engage with patients, families, and caregivers to ensure their needs and preferences are met in care planning.

Qualifications

  • Bachelor's degree in Healthcare Administration, Social Work, Nursing, or a related field.
  • Minimum of 3 years of experience in healthcare, community services, or a related sector.
  • Strong understanding of Advance Care Planning and palliative care principles.
  • Excellent communication, interpersonal, and stakeholder management skills.
  • Ability to work collaboratively in a multidisciplinary team environment.
  • Proficient in data analysis, report writing, and project management.
  • Passion for improving healthcare outcomes and advocating for patient-centered care.
  • Familiarity with Singapore's healthcare system and policies is a plus.
