Responsibility
- Support the development and implementation of Advance Care Planning (ACP) initiatives in Singapore.
- Collaborate with healthcare providers, community organizations, and government agencies to promote palliative care services.
- Assist in the design and execution of training programs for healthcare professionals and caregivers.
- Conduct research and analysis to identify gaps and opportunities in palliative care delivery.
- Coordinate outreach and awareness campaigns to educate the public on the importance of ACP and palliative care.
- Monitor and evaluate the effectiveness of programs and initiatives, providing recommendations for improvement.
- Prepare reports, presentations, and documentation for stakeholders and senior management.
- Engage with patients, families, and caregivers to ensure their needs and preferences are met in care planning.
Qualifications
- Bachelor's degree in Healthcare Administration, Social Work, Nursing, or a related field.
- Minimum of 3 years of experience in healthcare, community services, or a related sector.
- Strong understanding of Advance Care Planning and palliative care principles.
- Excellent communication, interpersonal, and stakeholder management skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Proficient in data analysis, report writing, and project management.
- Passion for improving healthcare outcomes and advocating for patient-centered care.
- Familiarity with Singapore's healthcare system and policies is a plus.
