Responsibility
- Assist patients with prescription-related inquiries and ensure accurate medication management.
- Conduct follow-ups on patient visits to monitor progress and address any concerns.
- Review and respond to non-urgent medical questions from patients in a timely manner.
- Analyze and interpret test results, providing clear explanations to patients and healthcare providers.
- Continuously monitor and manage patient in-baskets to ensure all inquiries are addressed promptly.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
- Provide education and support to patients on managing their health conditions.
Qualifications
- Active USRN (United States Registered Nurse) license, with New York state licensure preferred.
- Minimum of 2 years of clinical nursing experience in a healthcare setting.
- Strong knowledge of medical terminology, prescription management, and patient care protocols.
- Excellent communication skills, both written and verbal, with a patient-centered approach.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in using electronic health record (EHR) systems and other healthcare software.
