Responsibility
- Provide high-quality, compassionate nursing care to patients in their homes, ensuring adherence to clinical standards and organizational protocols.
- Assess patients' physical, emotional, and spiritual needs, developing and implementing individualized care plans.
- Administer medications, manage symptoms, and monitor patients' conditions, reporting any changes to the healthcare team.
- Educate patients and their families on disease management, medication administration, and end-of-life care options.
- Collaborate with physicians, social workers, and other healthcare professionals to ensure comprehensive and coordinated care.
- Maintain accurate and up-to-date patient records, documenting all care provided and observations made.
- Provide emotional and psychological support to patients and their families, fostering a compassionate and respectful environment.
- Participate in ongoing training and professional development to stay current with best practices in palliative and home care.
Qualifications
- Diploma or Bachelor's degree in Nursing from an accredited institution.
- Valid nursing license in Indonesia (STR) and certification in palliative or home care nursing is a plus.
- Minimum of 3 years of clinical nursing experience, with at least 1 year in home care, palliative care, or a related field.
- Strong clinical skills, including wound care, pain management, and symptom control.
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with patients and families.
- Compassionate, empathetic, and patient-centered approach to care.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in basic computer skills and electronic health record (EHR) systems.
