Responsibility
- Conduct comprehensive psychosocial assessments for patients and families in healthcare settings, including nursing homes and palliative care.
- Develop and implement individualized care plans to address emotional, social, and financial challenges faced by patients.
- Provide counseling and crisis intervention to patients and families coping with illness, grief, or end-of-life transitions.
- Collaborate with doctors, nurses, and other healthcare professionals to ensure integrated and patient-centered care.
- Advocate for patients’ rights and access to community resources, including financial aid, housing, and support services.
- Facilitate support groups and educational workshops for patients, families, and caregivers.
- Maintain accurate and confidential documentation of patient interactions, assessments, and interventions.
- Stay updated on best practices in medical social work and participate in continuous professional development.
Qualifications
- Bachelor’s or Master’s Degree in Social Work (accredited by a recognized institution).
- Minimum 3 years of experience as a Medical Social Worker, preferably in healthcare, palliative care, or community settings.
- Licensed or registered as a Social Worker (or equivalent certification in your country of practice).
- Strong knowledge of psychosocial assessment tools, crisis intervention, and care planning.
- Excellent communication and interpersonal skills, with the ability to build trust with diverse patient populations.
- Familiarity with healthcare systems, insurance processes, and community resources in Indonesia (or willingness to learn).
- Compassionate, empathetic, and resilient with a commitment to ethical social work practices.
- Proficiency in English (additional fluency in Bahasa Indonesia is a plus).
