Responsibility

  • Educate mild chronic renal failure and end-stage renal failure patients on self-care management, including dietary guidelines, medication adherence, and lifestyle modifications.
  • Develop and implement individualized care plans in collaboration with nephrologists, dietitians, and other healthcare professionals.
  • Monitor patient progress through regular assessments and adjust care plans as needed to optimize outcomes.
  • Facilitate smooth transitions between different stages of renal care, including pre-dialysis, dialysis, and post-transplant phases.
  • Provide emotional support and counseling to patients and their families to address the psychological and social impacts of renal disease.
  • Coordinate with healthcare teams to ensure continuity of care, including referrals to specialists and community resources.
  • Maintain accurate and up-to-date patient records in compliance with hospital policies and regulatory standards.
  • Conduct community outreach and educational programs to raise awareness about renal health and preventive care.

Qualifications

  • Bachelor’s degree in Nursing, Healthcare Management, or a related field (preferred).
  • Minimum of 2 years of experience in renal care, nephrology, or chronic disease management.
  • Certification in renal nursing or nephrology (e.g., Certified Nephrology Nurse - CNN) is a plus.
  • Strong knowledge of renal disease pathophysiology, treatment modalities, and patient education strategies.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and multidisciplinary teams.
  • Compassionate, patient-centered approach with a focus on improving quality of life for renal patients.
  • Fluency in English; proficiency in Bahasa Indonesia or other local languages is an advantage.
