Responsibility
- Review and audit OASIS assessments and home health documentation for accuracy, completeness, and compliance with regulatory requirements.
- Identify and report documentation discrepancies, errors, or inconsistencies, providing constructive feedback to clinical staff.
- Collaborate with healthcare teams to ensure documentation aligns with best practices and organizational policies.
- Utilize EMR systems to track, analyze, and report on documentation trends and quality metrics.
- Develop and implement training materials or guidelines to improve documentation practices among clinical staff.
- Participate in quality assurance initiatives, audits, and continuous improvement projects.
- Maintain up-to-date knowledge of healthcare regulations, including Medicare/Medicaid guidelines and accreditation standards.
- Provide support and guidance to clinical teams on documentation workflows and compliance-related inquiries.
Qualifications
- Active Registered Nurse (RN) license in good standing.
- Minimum of 2 years of clinical experience in home health, hospice, or a related healthcare setting.
- Experience with OASIS assessments and home health documentation requirements.
- Proficiency in using EMR systems (e.g., Epic, Cerner, or similar platforms).
- Strong analytical skills with keen attention to detail and accuracy.
- Excellent written and verbal communication skills for providing feedback and training.
- Ability to work independently in a remote setting while collaborating effectively with a dispersed team.
- Knowledge of healthcare regulations, including Medicare/Medicaid conditions of participation and accreditation standards.
