Responsibility
- Coordinate and monitor asthma patient care plans in collaboration with healthcare providers, ensuring adherence to the Singapore National Asthma Program guidelines.
- Facilitate communication between patients, families, and healthcare teams to optimize treatment outcomes and patient satisfaction.
- Track and document patient progress, including medication adherence, symptom management, and follow-up appointments.
- Organize and conduct virtual or in-person patient education sessions on asthma management, triggers, and preventive care.
- Identify and address barriers to care, such as access to medications, financial constraints, or cultural considerations.
- Collaborate with community health workers and social services to provide holistic support to patients and their families.
- Maintain accurate and up-to-date records in electronic health systems, ensuring compliance with data privacy regulations.
- Participate in regular team meetings, case reviews, and program evaluations to continuously improve care delivery.
Qualifications
- Bachelor’s degree in Nursing, Public Health, Social Work, or a related healthcare field (or equivalent experience).
- Minimum of 2 years of experience in care coordination, case management, or a similar role in healthcare.
- Strong understanding of asthma management, chronic disease care, or public health programs.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
- Proficiency in using electronic health records (EHR) and Microsoft Office Suite (Word, Excel, PowerPoint).
- Ability to work independently, manage time effectively, and prioritize tasks in a remote work environment.
- Cultural sensitivity and experience working with diverse populations.
- Certification in care coordination, case management, or asthma education (e.g., Certified Asthma Educator) is a plus.
