Responsibility
- Conduct comprehensive psychosocial assessments for nursing home residents to identify their emotional, social, and psychological needs.
- Develop and implement individualized care plans in collaboration with healthcare teams, residents, and their families.
- Provide counseling and emotional support to residents and their families, addressing issues such as grief, adjustment to nursing home life, and end-of-life planning.
- Advocate for residentsâ rights and ensure they receive appropriate medical, social, and community services.
- Coordinate with external agencies, such as government programs and non-profit organizations, to secure additional resources for residents.
- Facilitate support groups and educational workshops for residents and families on topics like aging, mental health, and caregiving.
- Maintain accurate and confidential records of resident interactions, assessments, and care plans in compliance with regulatory standards.
- Participate in interdisciplinary team meetings to review resident progress and adjust care plans as needed.
Qualifications
- Bachelorâs or Masterâs degree in Social Work, Psychology, or a related field. Certification or licensure in social work is preferred.
- Minimum of 2 years of experience as a Medical Social Worker, preferably in a nursing home, hospital, or geriatric care setting.
- Strong knowledge of psychosocial assessment tools, counseling techniques, and case management principles.
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with elderly residents and their families.
- Familiarity with local and international healthcare regulations, ethical standards, and patient rights.
- Ability to work collaboratively in a multidisciplinary team and manage multiple cases with compassion and professionalism.
- Proficiency in English; knowledge of Indonesian or other local languages is a plus.
- Commitment to continuous learning and staying updated on best practices in geriatric social work.
