Responsibility
- Conduct psychosocial assessments to identify patient and family needs.
- Provide counseling and emotional support to patients and their families.
- Develop and implement care plans in collaboration with healthcare teams.
- Advocate for patients' rights and access to necessary resources.
- Facilitate discharge planning and coordinate community-based support services.
- Educate patients and families on coping strategies and available social services.
- Maintain accurate and confidential patient records.
- Participate in case conferences and multidisciplinary team meetings.
Qualifications
- Bachelor's or Master's degree in Social Work or a related field.
- Licensed or certified as a Social Worker in Indonesia.
- Minimum of 2 years of experience in medical or healthcare social work.
- Strong interpersonal and communication skills.
- Ability to work collaboratively in a fast-paced healthcare environment.
- Knowledge of local social services and community resources.
- Compassionate and patient-centered approach.
- Fluency in English and Bahasa Indonesia.
