Responsibility
- Conduct comprehensive psychosocial assessments for patients and families to identify emotional, social, and financial needs.
- Develop and implement individualized care plans in collaboration with healthcare teams, patients, and caregivers.
- Provide crisis intervention and short-term counseling to patients facing acute medical or emotional challenges.
- Facilitate support groups and educational workshops on topics such as chronic illness management, grief, and mental health awareness.
- Advocate for patients' rights and access to resources, including financial assistance, housing, and community services.
- Coordinate discharge planning to ensure smooth transitions from hospital to home or other care settings.
- Maintain accurate and confidential documentation of patient interactions and progress in compliance with healthcare regulations.

Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited institution.
- Valid social work license (e.g., LSW, LMSW, or equivalent international certification).
- Minimum of 2 years of experience in medical social work, healthcare, or a related field.
- Strong knowledge of psychosocial assessment tools, crisis intervention techniques, and case management.
