Responsibility
- Review and verify medication orders for hospitalized patients to ensure accuracy, appropriateness, and safety
- Prepare and dispense medications according to established protocols and standards
- Conduct medication reconciliation for patients upon admission, transfer, and discharge
- Monitor drug therapy for efficacy, side effects, and potential interactions
- Provide drug information and education to healthcare professionals and patients
- Participate in multidisciplinary rounds to optimize patient medication regimens
- Maintain accurate and complete pharmacy records in compliance with regulations
- Assist in the development and implementation of pharmacy policies and procedures
- Perform quality assurance checks on pharmacy operations and medication storage
Qualifications
- Bachelor's degree in Pharmacy followed by a Professional Pharmacist degree (Apoteker)
- Active Pharmacist License (STRA) issued by the Indonesian Ministry of Health
- Minimum 1-2 years of experience in hospital pharmacy or inpatient care (fresh graduates with clinical rotations will be considered)
- Strong knowledge of pharmaceutical calculations, drug interactions, and therapeutic guidelines
- Excellent communication skills in English and Bahasa Indonesia
- Ability to work effectively in a fast-paced, team-oriented environment
- Familiarity with hospital pharmacy information systems and electronic medical records
- Commitment to continuous professional development and evidence-based practice
