Responsibility
- Review and verify inpatient medication orders for accuracy, appropriateness, and potential drug interactions or allergies.
- Collaborate with physicians, nurses, and healthcare teams to clarify ambiguous or irrational prescriptions and propose therapeutic alternatives.
- Conduct clinical interventions, including dose adjustments, drug substitutions, and IV-to-PO conversions, to optimize patient outcomes.
- Document all pharmacy-related activities, including confirmations with prescribers, in electronic health records (EHR) or paper charts as required.
- Monitor patient medication regimens for efficacy, safety, and adherence to hospital protocols and national guidelines.
- Educate patients and healthcare staff on proper medication use, side effects, and storage conditions.
- Participate in multidisciplinary rounds to provide real-time pharmaceutical input on complex cases.
- Ensure compliance with hospital policies, local regulations, and international pharmacy standards.
Qualifications
- Bachelor’s degree in Pharmacy (S.Farm) from an accredited institution; Apoteker license is mandatory.
- Minimum 2 years of experience as a Clinical or Inpatient Pharmacist in a hospital setting.
- Strong knowledge of pharmacotherapy, pharmacokinetics, and drug information resources.
- Proficiency in using hospital information systems (HIS) and pharmacy software.
- Excellent communication skills in English and Indonesian to liaise with international and local teams.
- Attention to detail and ability to work under pressure in a high-volume environment.
- Familiarity with Indonesian healthcare regulations and JKN (National Health Insurance) protocols is a plus.
