Responsibility
- Coordinate follow-up care for asthma patients enrolled in the Singapore National Asthma Program.
- Liaise with healthcare providers, patients, and families to ensure timely and effective communication.
- Monitor patient progress, track treatment adherence, and document outcomes in electronic health records.
- Organize and facilitate patient education sessions on asthma management and prevention.
- Collaborate with medical teams to develop and implement individualized care plans.
- Schedule and confirm follow-up appointments, ensuring minimal disruptions to patient care.
- Collect and analyze patient data to identify trends and areas for program improvement.
- Provide administrative support, including maintaining patient records and generating reports.
Qualifications
- Diploma or Bachelor’s degree in Nursing, Healthcare Administration, Public Health, or a related field.
- Minimum 2 years of experience in healthcare coordination, patient care, or a similar role.
- Strong knowledge of asthma management, respiratory care, or chronic disease programs.
- Excellent communication and interpersonal skills to engage with patients and healthcare professionals.
- Proficient in using electronic health record (EHR) systems and Microsoft Office Suite.
- Ability to work independently and as part of a multidisciplinary team in a fast-paced environment.
- Highly organized with strong attention to detail and problem-solving abilities.
- Fluency in English; additional languages (e.g., Indonesian, Mandarin) are a plus.
