Responsibility
- Coordinate and oversee the clinical care pathways for cancer patients, ensuring timely and efficient delivery of treatment plans.
- Collaborate with oncologists, nurses, and other healthcare professionals to develop and implement individualized patient care plans.
- Monitor patient progress and adjust care plans as necessary to optimize outcomes and patient satisfaction.
- Facilitate communication between patients, families, and healthcare teams to ensure clarity and continuity of care.
- Manage patient scheduling, referrals, and follow-ups to minimize delays and improve operational efficiency.
- Ensure compliance with clinical protocols, regulatory standards, and best practices in oncology care.
- Identify opportunities for process improvements and implement strategies to enhance patient care and operational workflows.
- Provide training and support to clinical staff on care coordination tools and systems.
Qualifications
- Bachelor’s degree in Nursing, Healthcare Administration, or a related field. A Master’s degree is a plus.
- Minimum of 3-5 years of experience in clinical care coordination, preferably in oncology or a specialized healthcare setting.
- Strong understanding of clinical workflows, patient care pathways, and healthcare regulations.
- Excellent communication and interpersonal skills, with the ability to build relationships with patients, families, and healthcare teams.
- Proficient in using electronic health records (EHR) and care coordination software.
- Demonstrated ability to manage multiple priorities, solve problems, and work effectively in a fast-paced environment.
- Certification in care coordination or oncology nursing (e.g., OCN) is highly desirable.
- Fluency in English and Bahasa Indonesia is required; additional languages are a plus.
