Responsibility

  • Coordinate and oversee the clinical care pathways for cancer patients, ensuring timely and efficient delivery of treatment plans.
  • Collaborate with oncologists, nurses, and other healthcare professionals to develop and implement individualized patient care plans.
  • Monitor patient progress and adjust care plans as necessary to optimize outcomes and patient satisfaction.
  • Facilitate communication between patients, families, and healthcare teams to ensure clarity and continuity of care.
  • Manage patient scheduling, referrals, and follow-ups to minimize delays and improve operational efficiency.
  • Ensure compliance with clinical protocols, regulatory standards, and best practices in oncology care.
  • Identify opportunities for process improvements and implement strategies to enhance patient care and operational workflows.
  • Provide training and support to clinical staff on care coordination tools and systems.

Qualifications

  • Bachelor’s degree in Nursing, Healthcare Administration, or a related field. A Master’s degree is a plus.
  • Minimum of 3-5 years of experience in clinical care coordination, preferably in oncology or a specialized healthcare setting.
  • Strong understanding of clinical workflows, patient care pathways, and healthcare regulations.
  • Excellent communication and interpersonal skills, with the ability to build relationships with patients, families, and healthcare teams.
  • Proficient in using electronic health records (EHR) and care coordination software.
  • Demonstrated ability to manage multiple priorities, solve problems, and work effectively in a fast-paced environment.
