Responsibility
- Provide professional nursing care to patients in their homes, including wound care, medication administration, and vital sign monitoring.
- Develop and implement personalized care plans in collaboration with physicians and family members.
- Assess patient health status and report any changes to the healthcare team promptly.
- Educate patients and caregivers on disease management, nutrition, and rehabilitation exercises.
- Maintain accurate and up-to-date medical records and documentation.
- Coordinate with multidisciplinary teams to ensure seamless continuity of care.
- Offer emotional support and counseling to patients and their families.
- Adhere to infection control protocols and maintain a safe home care environment.
Qualifications
- Valid nursing license (STR) and registration with the Indonesian Nurses Association (PPNI).
- Minimum 2 years of experience in home care, geriatric nursing, or inpatient ward settings.
- Strong clinical skills in patient assessment, care planning, and emergency response.
- Excellent communication and interpersonal skills to build trust with patients and families.
- Ability to work independently with minimal supervision in diverse home environments.
- Proficiency in basic medical equipment and digital health record systems.
