Responsibility
- Develop and implement individualized care plans for patients with mental health conditions, ensuring alignment with clinical goals and patient preferences.
- Coordinate with psychiatrists, psychologists, social workers, and nurses to deliver integrated, multidisciplinary care.
- Monitor patient progress through regular assessments and adjust care plans to optimize recovery outcomes.
- Facilitate discharge planning and community reintegration, linking patients with support services, housing, and vocational rehabilitation.
- Advocate for patient rights and access to resources, ensuring equitable and culturally sensitive care.
- Maintain accurate and confidential case documentation in compliance with hospital and regulatory standards.
- Conduct psychoeducation sessions for patients and families to enhance understanding of mental health conditions and coping strategies.
- Participate in quality improvement initiatives to enhance case management protocols and service delivery.
Qualifications
- Bachelor’s degree in Psychology, Social Work, Nursing, or a related field; Master’s degree preferred.
- Minimum 3 years of experience in case management, mental health care, or a similar role.
- Strong knowledge of mental health disorders, treatment modalities, and recovery models.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively in a multicultural and interdisciplinary team.
- Proficiency in case management software and Microsoft Office Suite.
- Certification in case management (e.g., CCM, ACM) is a plus.
- Fluency in English; knowledge of Indonesian or other local languages is advantageous.
