Responsibility
- Coordinate and manage patient care plans in collaboration with healthcare providers to ensure timely and effective treatment.
- Assist patients with scheduling appointments, follow-ups, and referrals to specialists or other healthcare services.
- Maintain accurate and up-to-date patient records, ensuring compliance with healthcare regulations and confidentiality standards.
- Act as a liaison between patients, families, and healthcare teams to facilitate clear communication and address concerns.
- Monitor patient progress and provide support in navigating healthcare systems, including insurance and financial assistance programs.
- Organize and participate in multidisciplinary team meetings to review patient cases and adjust care plans as needed.
- Provide administrative support to polyclinic staff, including data entry, report generation, and coordination of medical supplies.
- Educate patients and their families on health management, preventive care, and available community resources.
Qualifications
- Diploma or degree in Nursing, Healthcare Administration, Social Work, or a related field.
- Minimum of 2 years of experience in a healthcare or community services role, preferably in care coordination or patient advocacy.
- Strong organizational and multitasking skills, with the ability to prioritize tasks in a fast-paced environment.
- Excellent communication and interpersonal skills, with fluency in English (additional languages are a plus).
- Proficiency in using healthcare management software, electronic health records (EHR), and Microsoft Office Suite.
