Responsibility
- Coordinate and monitor patient care plans in collaboration with healthcare providers, ensuring alignment with medical recommendations.
- Liaise between patients, families, and healthcare teams to facilitate clear communication and understanding of treatment goals.
- Assist in organizing home care services, including medical equipment, nursing support, and therapy sessions.
- Conduct regular follow-ups with patients and families to assess progress, address concerns, and adjust care plans as needed.
- Maintain accurate and up-to-date patient records, documenting care coordination activities and outcomes.
- Educate patients and families on available healthcare resources, insurance coverage, and community support programs.
- Advocate for patient needs by identifying gaps in care and recommending solutions to improve service delivery.
- Participate in multidisciplinary team meetings to review patient cases and contribute to care strategy discussions.
Qualifications
- Diploma or Bachelor’s degree in Nursing, Healthcare Administration, Social Work, or a related field.
- Minimum 2 years of experience in healthcare coordination, patient advocacy, or a similar role.
- Strong communication and interpersonal skills, with the ability to build trust with patients and healthcare professionals.
- Familiarity with medical terminology, care planning, and healthcare systems in Indonesia.
- Proficient in documentation and record-keeping, with attention to detail and confidentiality.
- Ability to work independently and collaboratively in a fast-paced, dynamic environment.
- Compassionate and patient-centered approach with a commitment to ethical healthcare practices.
- Fluency in English and Bahasa Indonesia is highly preferred.
