Responsibility
- Develop and deliver training programs for Community Nursing Teams on Advance Care Planning (ACP) best practices.
- Collaborate with the Palliative Care Team to align ACP strategies with patient-centered care goals.
- Create educational materials, workshops, and resources to support ACP implementation.
- Monitor and evaluate the effectiveness of ACP training programs, making data-driven improvements.
- Provide guidance and mentorship to nursing staff on sensitive ACP conversations.
- Ensure compliance with healthcare regulations and ethical standards in ACP documentation.
- Facilitate interdisciplinary meetings to discuss ACP progress and challenges.
- Contribute to research and quality improvement initiatives related to end-of-life care.
Qualifications
- Bachelor's degree in Nursing, Healthcare Administration, or a related field.
- Minimum of 3 years of experience in healthcare, with a focus on palliative care or ACP.
- Strong understanding of Advance Care Planning principles and practices.
- Excellent communication and interpersonal skills for training and mentorship.
- Ability to work independently and collaboratively in a part-time capacity.
- Familiarity with healthcare regulations and ethical considerations in ACP.
- Experience in developing educational materials or training programs is a plus.
- Proficiency in English; additional languages are advantageous.
