Responsibility
- Assess and evaluate patients’ medical, psychological, and social needs to develop individualized care plans.
- Coordinate with healthcare providers, insurance companies, and community resources to ensure timely and appropriate care.
- Monitor patient progress, adjust care plans as needed, and document all interventions and outcomes.
- Advocate for patients’ rights and needs, ensuring they receive the highest standard of care and support.
- Educate patients and families on treatment options, resources, and self-management strategies.
- Collaborate with multidisciplinary teams to facilitate smooth transitions between care settings (e.g., hospital to home).
- Maintain accurate and confidential patient records in compliance with healthcare regulations.
- Participate in case reviews, team meetings, and continuous professional development.
Qualifications
- Bachelor’s degree in Nursing or a related healthcare field (required).
- Active nursing license or relevant healthcare certification (preferred).
- Minimum 1 year of clinical experience (e.g., hospital, clinic, or community health).
- Strong communication, empathy, and problem-solving skills.
- Ability to work independently and manage multiple cases efficiently.
