Responsibility
- Coordinate and facilitate care plans for asthma patients in collaboration with healthcare providers.
- Monitor patient progress and adjust care strategies to optimize treatment outcomes.
- Liaise between medical teams, patients, and external stakeholders to ensure smooth care transitions.
- Maintain accurate patient records and documentation in compliance with institutional policies.
- Educate patients and families on asthma management, prevention, and lifestyle adjustments.
- Organize and participate in multidisciplinary team meetings to review patient cases.
- Identify gaps in care delivery and propose actionable solutions to enhance service quality.
- Support the implementation of healthcare programs and initiatives aimed at improving asthma care.
Qualifications
- Diploma or Bachelor’s degree in Nursing, Healthcare Administration, or a related field.
- Minimum 2 years of experience in healthcare coordination, patient care, or a similar role.
- Strong knowledge of asthma management protocols and respiratory care best practices.
- Excellent communication and interpersonal skills to engage with diverse stakeholders.
- Proven ability to manage multiple priorities in a fast-paced healthcare environment.
- Familiarity with electronic health record (EHR) systems and medical terminology.
