Responsibility

  • Conduct home visits to assess the health, developmental, and social needs of children aged 0-3 and their families.
  • Provide personalized health education on nutrition, hygiene, immunization, and early childhood development milestones.
  • Screen for developmental delays, health risks, and social determinants of health, referring families to appropriate services as needed.
  • Collaborate with pediatricians, social workers, and community partners to create tailored care plans for at-risk families.
  • Document all visits, interventions, and outcomes in compliance with program standards and hospital protocols.
  • Facilitate parenting workshops and support groups to empower caregivers with skills for nurturing child development.
  • Monitor and evaluate program effectiveness, contributing to data collection and reporting for continuous improvement.
  • Advocate for family-centered care and bridge gaps in access to healthcare and social services.

Qualifications

  • Diploma or Bachelor’s degree in Nursing, Social Work, Public Health, or a related field. Registered Nurse or Social Worker certification is a plus.
  • Minimum 2 years of experience in community health, maternal/child health, or early intervention programs.
  • Strong interpersonal and communication skills to build trust with diverse families and stakeholders.
  • Knowledge of child development stages (0-3 years) and evidence-based home visitation models (e.g., Nurse-Family Partnership).
  • Ability to work independently in the field while adhering to program guidelines and ethical standards.
  • Fluency in English; proficiency in Bahasa Indonesia or local dialects is advantageous.
  • Comfortable with digital tools for documentation, data entry, and telehealth support.
  • Passion for community service and a commitment to equity in healthcare access.
