Responsibility
- Coordinate and manage patient care plans, ensuring alignment with medical guidelines and individual needs.
- Collaborate with healthcare providers, insurance teams, and patients to facilitate high-quality care delivery.
- Conduct regular assessments of patient health status and adjust care plans as necessary.
- Educate patients and families on treatment options, preventive care, and wellness strategies.

Qualifications
- Active USRN (United States Registered Nurse) license required.
- Minimum of 3 years of clinical nursing experience, preferably in care management or a related field.
