Responsibility
- Coordinate patient care plans by collaborating with healthcare providers, patients, and families to ensure comprehensive and continuous care.
- Assess patient needs and develop individualized care plans that align with preventive health strategies and treatment goals.
- Facilitate communication between patients, doctors, nurses, and social services to streamline care delivery and resolve any barriers.
- Monitor patient progress and adjust care plans as needed, ensuring adherence to medical recommendations and preventive care guidelines.
- Educate patients and families on health management, preventive care practices, and available community resources.
- Maintain accurate and up-to-date patient records, documenting care plans, interventions, and outcomes in compliance with healthcare regulations.
- Advocate for patients’ rights and needs, ensuring they receive equitable and high-quality healthcare services.
- Participate in multidisciplinary team meetings to review patient cases, share insights, and improve care coordination strategies.
Qualifications
- Bachelor’s degree in Nursing, Public Health, Social Work, or a related healthcare field. Advanced degrees or certifications in care coordination are a plus.
- Minimum of 2-3 years of experience in healthcare, patient advocacy, or care coordination, preferably in a community or hospital setting.
- Strong knowledge of healthcare systems, preventive care principles, and patient-centered care models.
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with patients, families, and healthcare teams.
- Proven organizational and time-management skills, with the ability to manage multiple patient cases efficiently.
- Familiarity with electronic health records (EHR) and healthcare documentation standards.
- Compassionate and empathetic approach to patient care, with a commitment to ethical and culturally sensitive practices.
- Fluency in English; proficiency in Indonesian or other local languages is advantageous.
